The Opportunity

The Behavioural Support Coordinator supports the Social Worker, Assistant Director of Care, Director of Care, Nurse Managers, residents, and their family members. The Coordinator works within the guidelines of the Central East LHIN Behavioural Support Ontario Project to coordinate care and services for residents with responsive behaviours.

What Is In It For You
  • Utilizes effective communication and interpersonal skills when interacting with residents, family members, and care partners.
  • Ensures current care plans are in place for residents with responsive behaviours, guiding staff in providing care and mitigating risks.
  • Liaises with external partners to support resident-centered care.
  • Conducts resident case reviews to support care plans for residents with responsive behaviours.
  • Responds to concerns from residents or family members and takes appropriate action or escalates as needed.
  • Participates in quality improvement activities and reports monthly statistics on responsive behaviours.
  • Attends the Professional Advisory Committee quarterly.
  • Collaborates with pharmacists and physicians, maintaining knowledge of medication impacts on resident care.
  • Demonstrates understanding and commitment to the residence's philosophy, objectives, policies, and the Behavioural Support Ontario Project.
What You Will Need
  • Knowledge of dementia care, delirium, mental health issues, and their impact on residents and families.
  • Valid RPN certification, qualifications, and good standing with the College of Nurses of Ontario.
  • Specialized qualifications relevant to this position.
