The APh Clinical Pharmacist is responsible for providing prescribed medications, drugs and other pharmaceuticals as needed for adequate patient care, according to professional standards and practices. In addition, the APH Clinical Pharmacist is responsible for providing patients and providers with Therapeutic assessments and selected disease management through Medication Therapy Management program, working toward improving patient and practice outcomes. These Pharmacists function as provider delegates with outside agencies such as pharmacies and insurances. An advanced practice pharmacist is a licensed pharmacist who has been recognized by the board, and is entitled to practice advanced practice pharmacy, within or outside of a licensed pharmacy. The advanced practice pharmacist license shall be coterminous (concurrent) with the licensee’s pharmacist license. This is a part time position, working 20 hours per week, schedule to be determined.
The pay range for this role is $62 - $93 on an hourly basis.
