Job summary

A healthcare organization specializing in dementia care in Singapore is seeking a passionate individual to conduct assessments and develop care plans for persons with dementia. The ideal candidate will have a degree in an allied health profession and possess excellent communication skills. Responsibilities include providing interventions, collaborating with professionals for holistic care, and facilitating caregiver support groups. Clinical experience of at least 2 years is preferred.

Qualifications

  • Preferably 2 years of clinical experience in a relevant field.
  • Able to work on occasional Saturdays and weekday evenings.
  • Prior working experience in dementia care is an advantage.

Responsibilities

  • Conduct case assessments and formulate Individualized Care Plans.
  • Provide interventions such as psychoeducation and supportive counselling.
  • Liaise with multidisciplinary professionals for holistic care.
  • Assist in preparing data for programme evaluation.
  • Plan and facilitate Caregiver Support Group sessions.

Responsibilities
  • Conduct case assessment and formulate Individualized Care Plan for Persons with Dementia and their carers.
  • Provide intervention such as psychoeducation, supportive counselling and service referral. Able to administer Advance Care Planning will be a bonus.
  • Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
  • Provide case recordings and relevant documentations.
  • Participate in team meetings and case conferences as required.
  • Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
  • Plan and facilitate Caregiver Support Group sessions.
  • Support in outreach events and talks.
  • Any ad-hoc tasks as and when required.
Requirements
  • Preferably with degree in an allied health profession/social work/ psychology etc. Senior position may be considered for this role.
  • Preferably 2 years of clinical experience.
  • Good oral, written and interpersonal communications skills.
  • Ability to facilitate group sessions will be an added advantage.
  • Able to work on occasional Saturdays and weekday evenings.
  • Passionate about dementia care and working with people.
