Institution: National University Health System

As part of the multi-disciplinary team with the National University Health System (NUHS)@Home, you will provide quality pharmaceutical care as our Pharmacist.

Job Responsibilities
  1. Review transfer medications appropriately, contact patients pre-transfer on medication supply needed, and answer medicine related questions.
  2. Medication counselling on transfer medications by a variety of means (in-person and virtual).
  3. Review discharge prescriptions appropriately.
  4. Verify new orders by the physicians and identify inappropriate medication/doses if relevant.
  5. Assist with performing medication supply duty.
  6. Follow up on medication preparation status.
  7. Undertake duties which may be assigned by the NUHS staff member depending on the requirements.
Requirements
  1. Bachelor of Science in Pharmacy from an accredited college by Singapore Pharmacy Council.
  2. At least 2 years of relevant experience.
  3. Fully registered with the Singapore Pharmacy Council.
  4. Possess excellent communication, interpersonal and organisation skills.
  5. Able to work independently and in a team.
  6. Weekdays and/or weekends working hours are available.
