Overview

About Healthcare Support Department: Healthcare Support Department supports frail/home-bound elderly in their healthcare needs so that they can remain to age well at the comfort of their homes and in the community. The department understands the challenges faced in taking care of elderly at home. They ensure there is continuity of care integrating healthcare and social support whenever necessary so that caregivers and their loved ones will be well-supported and enjoy good care at the same time.

Responsibilities
  • Conduct home visit to render occupational therapy to clients in the premise of their own homes
  • Collaborate closely with multidisciplinary teams, caregivers, and internal or external partners to support clients’ overall well-being.
  • Perform thorough client assessments, develop care plans, and maintain accurate documentation in compliance with professional and organisational standards.
  • Deliver evidence-based individualised care through holistic comprehensive assessment, treatment and evaluation of care.
  • Maintain high quality patient documentation and records.
  • Coordinate locum occupational therapy services for clients.
  • Manage and supervise a team of Healthcare Assistants who provide Home Personal Care service to clients.
Requirements
  • Degree in Occupational Therapy from an accredited institution.
  • Must be fully registered with Allied Health Professions Council.
  • At least 2 years of experience in an acute hospital setting or community setting.
  • At least 2 years of leadership experience in managing teams and providing supervision
  • Passionate to work with the elderly and their caregivers
  • Comfortable conducting therapy in clients’ homes, with adaptability to different environments.
  • Meticulous and good time management
  • Strong organisational skills and attention to detail in documentation and reporting.
  • Able to thrive in a fast-paced, dynamic, and multifaceted work setting.
  • Good interpersonal and communication skills, with the ability to build rapport with clients and their families, as well with colleagues and external stakeholders.
  • Proficient in basic IT applications for documentation and scheduling.
  • This position is on contract basis.
