A prominent healthcare provider in Wales is looking for a dedicated Band 3 Nursing Support Worker to join the Care at Home team. You will provide essential care to patients including end-of-life care, ensuring individualized and effective standards of care. This role requires effective communication and the flexibility to meet service needs. Community experience is desirable, training will be provided.
Pembrokeshire Intermediate Care Team are offering an exciting opportunity for a motivated, enthusiastic and passionate Band 3 NursingSupport Worker to join our Care at Home team.
The successful candidate will be competent to work without direct supervision, within a framework that has been agreed, to provide high quality care to patients including end of life, therefore reducing the need for acute hospital admission and facilitating earlier discharges.
As part of the Care at Home Team you will ensure that patients receivesafe, individualised and effective standards of care by following care plansand treatment interventions as directed by the designated Coordinator.
The successful candidate will need to communicate effectively with otherteam members at all times across all of the integrated teams anddemonstrate a flexible approach to the needs of the service.
Community experience is desirable, but not essential as induction andtraining will be provided.
The ability to travel in a timely manner across the Health Board is essential.
The ability to speak Welsh is desirable for this post; English and/or Welshspeakers are equally welcome to apply.
