Home Health Clinician — Discharge & Assessment Expert
Descripción de la vacante
A regional health trust in the United Kingdom is looking for a full-time Technical Instructor for their Discharge Home to Assess Team. This role involves providing clinical support to patients in their homes, maintaining treatment records, and participating in health promotion initiatives. Ideal candidates will have an NVQ 3 or equivalent in health-related qualifications. The position offers flexible working options and a supportive environment.

- The ideal applicant would have a health-related qualification to the level of NVQ 3 or equivalent community experience.
Responsabilidades
- Provide support, assessment and treatment for patients within their own homes.
- Maintain patient treatment records using System One.
- Participate in clinical supervision and service-level education.
- Manage a caseload of non-complex patients and prioritize tasks.
