A local healthcare practice in Yelverton is seeking a Care Coordinator to support individuals in accessing necessary services. The role includes managing online consultation triage, assisting the clinical team, and handling various administrative tasks. The ideal candidate should possess strong IT and communication skills, attention to detail, and experience in a general practice setting. This is a crucial role in ensuring a smooth patient experience.
We are looking for an organised, compassionate, and proactive Care Coordinator to join our team. In this role, you will play a key part in supporting individuals to access the right services at the right time, ensuring their care is well-planned, person-centred, and responsive to their needs. The role will primarily focus on triaging GP online consultation and telephone triage requests, supporting the clinical team through prescribing clerk duties, and assisting with wider administrative and reception responsibilities.
Administration duties include scanning and filing incoming post aswell as electronic clinical letters.
Deal with incoming practice email queries directly fromhospitals or care agencies.
Assist with signposting patients to appropriate community orlocal services, alongside social prescribers.
Act on communications from hospitals or community providers.
Action and follow death protocol procedure allowing a smoothprocess for families or close relatives.
Manage incoming medical reports and SAR requests (medicalrecord requests) from patients and third parties.
Assist with and ensure patients participate in local ornational screening programmes.
Pro-actively ensure patients participate in vaccinationopportunities such as flu, shingles, or childhood immunisations.
Organise and prioritise own workload.
This is a patient-facing non-clinical role that requiresexcellent organisational skills, attention to detail, and a good understandingof General Practice processes. Experience of working in a GP surgery andexperience with clinical triage systems is desirable.
