The TRFT Adult Speech & Language Therapy Service is an integrated service assessing and treating community patients in a variety of settings: in their own homes, care homes and outpatient clinics; as inpatients at Rotherham General Hospital; as outpatients along Head & Neck Cancer regional pathways; and in addition providing a Voice and Upper Airway Service working closely with ENT colleagues. We are a small, close-knit team looking for an experienced Clinical Specialist Speech & Language Therapist to lead the care homes provision.For the right candidate this may also be suitable as a fantastic development opportunity as a Band 6 Development post. On successful completion of the competencies outlined in their development plan, the candidate would progress on to full Band 7 duties and pay. As part of a Band 6 Development post, access to regular supervision from Clinical Specialists will be arranged

The successful applicant will triage and prioritise care home referrals taking into account urgency, clinical acuity and location. The post holder will hold their own caseload assessing and managing residents who are experiencing swallowing and communication problems in the context of highly complex needs related to a range of acquired and/or progressive neurological or long term conditions including dementias and respiratory disorders. They will link in with the MDT around care homes and the wider community to ensure the best outcomes for residents. As a key contact for care homes and the MDT around them, they will be a source of knowledge and clinical expertise and the role will include providing education and training programmes in response to local need.They will oversee, review, and ensure application of, relevant local guidance eg eating & drinking with acknowledged risk, end of life care, in keeping with national frameworks and other regional services.They will work with the other service leads to oversee the supervision and development of relevant competencies for community based speech and language therapists. They will lead and/or support other team members and the wider care homes MDT in the development of existing and new treatments and evaluate outcomes.The post-holder will deputise for the Clinical Lead at times of leave. Due to the size of the team there will be a requirement to occasionally provide cover on the wards at Rotherham Hospital

• Knowledge and experience of using standardised assessments and development of relevant outcome measures in swallowing/communication disorders, particularly with respect to acquired and progressive neurological conditions including dementias and respiratory disorders in a community setting including care homes
• Demonstrates knowledge of relevant equipment/resources available including assistive technology and therapy software and able to assess and fit where applicable
• Knowledge and understanding of relevant national legislation, guidance and regulatory framework including NSF, NICE, Information Governance, Equality and Diversity, Data Protection, Safe Guarding, Professional and Clinical Guidelines with particular relation to acquired and progressive neurological and respiratory conditions including dementias and care homes
• Knowledge of Clinical Governance and implications for service area
• Knowledge and evidence of participation in clinical audit and evidence gathering methods
• Knowledge and understanding of Health & Safety in the work environment and implications for service area
• Awareness and understanding of relevant health and Social Care legislation eg MCA 2015
• Evidence of leadership expertise and knowledge acquired through CPD and experience (eg via leading on project area/staff/clinical specialism)

• Clinical experience of working with complex patients with swallowing and communication problems particularly in outpatient/ community settings including care homes
• Experience of management of resources
• Evidence of development and supervision of staff and students
• Up to date documented evidence of CPD and KSF relevant to clinical area

• Current HCPC Registration as a Speech & Language Therapist

• Recognised Speech & Language Therapy Diploma/Degree qualification or equivalent
• Due to the nature of the role it is essential that applicants are able to travel independently between TRFT sites and for patient visits in the Rotherham area and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010
• Post-graduate qualifications to Masters level or equivalent with respect to the assessment and management and treatment of communication and swallowing disorders for acquired and progressive neurological conditions including dementias
• Membership of Professional body- RCSLT
• Management/leadership qualification or equivalent
• Completed recognised IT course e.g. ECDL/Elite/MOST
