Job summary

Qualifications

Responsibilities
- Administer medicines and conduct medication reviews.
- Liaise with community pharmacies for medication synchronization.
- Collaborate with Clinical Pharmacists on complex cases.

- Undertake patient-facing and patient-supporting roles
- Ensure effective medicines use through shared decision-making conversations
- Administer medicines, including checking inhaler technique
- Support medication reviews and medicines reconciliation
- Utilize consultation skills for partnership with patients
- Support Clinical Pharmacist in Structure Medication Reviews (SMR)
- Provide expertise for public health and social care needs
- Manage shared care protocols and liaise with Clinical Pharmacists
- Support the PCN to deliver on QOF, IIF, and other enhanced services.
