An excitingand rare opportunity has arisen for anOccupational Therapistto jointhe Post Covid multidisciplinary Team. The role will contribute in the delivery of high standards of health care to patients with post covid syndrome both remotely and in face to face clinics across the area of Norfolk and Waveney. Experience with working with patients who have long term health conditions is preferred

We are looking for the right people for our team, who have a passion for patient centred, supportive care within a multi-professional team. This role will offer you opportunities to carry out complex, patient focused assessment, health coaching and teach self-management techniques. You will increase your understanding of physical health and mental wellbeing, MDT collaboration and the possible symptoms and complications for patients in Post-Covid syndrome.

Main duties of the job

To perform as an Occupational Therapist, performing assessments, supporting patients with management of Post-COVID syndrome using local and National guidance.

The role includes;

  • Working as a resource for patients, their carers and other staff.
  • Working and liaising with the multi-disciplinary team (MDT) to provide high quality, evidence-based patient assessment and care.
  • Supporting the planning, co-ordinating, delivering and evaluating of care given to patients with Post-COVID syndrome in the community.
  • Support the development of the service across Norfolk and Waveney and work collaboratively with the wider MDT teams to promote a seamless service across the patient pathway.
  • Working autonomously within own scope of practice under the guidance of the Team Lead for the Post-COVID Assessment Service.

Constantly striving to improve care for patients with Post-COVID syndrome. The role requires engagement with other healthcare professionals to support the modernisation of care delivery with supervision from the Team Lead for the Post-COVID Assessment Service.

Job responsibilities
  • Provide assessment for patients referred into the service in line with agreed guidelines.
  • To liaise with GPs and Consultants about the patients under their care.
  • To accept referrals and manage a patient caseload according to agreed guidelines.
  • Supporting the improvement of Standards of Care for people with Post-COVID syndrome, in line with NICE
  • Working with the wider MDT across Norfolk and Waveney acute and community pathways to deliver care for people with Post-COVID syndrome.
  • To assist in the provision of training and education for patients, carers and professionals
  • Demonstrate specialist knowledge and skills through continuing professional development
  • To maintain own levels of clinical competence.
  • Working autonomously with support from Band 7 and Clinical lead and being accountable for own practice
  • To promote and facilitate research and evidence-based practice
  • To maintain a high profile in the clinical area, acting as a role model for other health care professionals.
  • To deliver and develop post covid interventions in a group setting primarily online.
  • Develop knowledge and skills in management and leadership.
  • Responsible for own workload and of other groups of staff within the team including work allocation, performance / attendance issues, appraisal and development where required.
  • Ensure timely and effective communication with the Clinical Team lead on all professional matters
  • To support maintenance of appropriate data for local and national reporting.
  • To maintain records, as required.
  • To use research findings critically in order to develop practice with supervisions from Clinical lead as required.
  • To provide information about Post-COVID syndrome to people, including its treatment and symptom management.
  • To provide information, education including offering placement opportunities for students, training to other healthcare professionals about Post-COVID syndrome and its assessment and management
  • Maintain links with other professionals and networks in order to learn and share examples of good practice.
  • To be responsible for developing and sustaining own knowledge, clinical skills and professional awareness and to maintain a professional profile.
  • To participate in continuing professional development activity and all mandatory training as required.
  • Accountable for ensuring workload of self and others is prioritised, planned and delivered in line with key performance indicators and service targets.
  • To deliver patient care across Norfolk and Waveney, face to face in a clinic setting, and via telephone, groups and virtual consultations

Qualifications
  • Current HCPC registration
  • Relevant clinical training

Experience

  • Experience with supporting patients' mental wellbeing.

  • Experience in presenting information to patients & carers
