Team Manager Keane Premier Support Services

As part of a multidisciplinary team, the post holder will have responsibility for assessment of care needs and the development of care plans & risk assessments, and the implementation and evaluation of these programmes ensuring the delivery of high‑quality care and outcomes for service users.

The post holder is expected to carry out all relevant forms of care without direct supervision, may be required to demonstrate procedures to and supervise SSSC registered care staff, and will direct, coordinate and supervise the work of other staff within their designated team to promote best practice in relation to People we Support.

SCOPE AND RANGE OF ROLE
  • Provide high quality, safe and supportive care to people we support within their own home, meeting identified physical, psychosocial and spiritual needs.
  • Supervise and teach care staff, providing mentorship, guidance, induction and orientation to the service.
  • Ensure that your designated runs are effectively managed in partnership with the Support Workers.
  • Carry out all relevant forms of care without direct supervision.
  • Promote self‑care, enabling people with long term conditions to live as independently as possible in their communities by ensuring that appropriate services and support are available to the People we Support and their family.
  • Facilitate guidance, mentorship and supervision of more junior staff within the service, developing and maintaining the ethos of the learning organisation.
  • Be responsible for assessment of care needs and the development of care plans and risk assessments, and the implementation, delegation and evaluation of these. Ensure that initial support plan is implemented with 4 weeks of receiving summary of needs.
  • Continually develop and maintain care plans, arrange and lead 6 monthly care reviews.
  • Ensure the delivery of high‑quality outcome focused care for People we support.
  • May be required to demonstrate procedures to and supervise SSSC registered care staff and provide induction and orientation to the service.
  • Ensure that your allocated team/run has an adequate supply of PPE.
  • Manage the rota for your team, to ensure effective use of resources.
  • Lead and support staff to ensure that mandatory training levels are maintained.
  • Act as a role model of the company and promote the company values in their actions.
MAIN DUTIES/RESPONSIBILITIES
  • Assess, plan, develop, implement and evaluate programmes of care for people we support to ensure delivery of a high standard of outcome focused care.
  • Maintain effective communications with people we support, relatives and other members of the multidisciplinary team to ensure that appropriate information is shared, and the person we support needs are met.
  • Be knowledgeable of all relevant Corporate, Local and National policies, procedures, standards and protocols.
  • Work within, and adhere to, defined Corporate, Local and National policies, procedures, standards and protocols.
  • Be fully compliant with the SSSC Code of Conduct and other relevant guidance documents, which support and maintain standards of professional practice.
  • Organise own and more junior staff workload and utilise time management skills to ensure that the interests of the people we support are met.
  • Lead a team of staff within a designated area and work using own initiative.
  • Maintain people we support’s care files within agreed standards.
  • Work as an integral member of the multidisciplinary team to ensure the highest and safest standard of quality outcome focused care is achieved.
  • Communicate efficiently and effectively any changes to the service or support to staff and the People we Support, creating and implementing effective communication processes for their area of responsibility.
  • Be willing to challenge and have difficult conversations where appropriate.
  • Be knowledgeable of all relevant equipment in use within the service.
  • Mentor and organise more junior members of staff to participate in available service training and continuing education programmes.
  • Participate in audit and research as part of the wider quality assurance strategy.
  • Provide encouragement and motivation to staff acting in an advocacy role to ensure that the people we support are at the centre of all care delivery.
  • Promote people’s rights to be respected and valued, effective outcome focused care and treatment, social inclusion, respect for families/carers, equality and respect for diversity, carers as a partner in care and management, and provide access to appropriate sources of information to help the person we support and carers reach informed, confident and safe decisions.
  • Be a continued accessible resource for people we support and carers acting as a link for other professionals.
  • Take responsibility for staff and ensure they are given appropriate support and guidance, providing motivational and constructive feedback to develop their team.
  • Demonstrate leadership, critical analysis and decision‑making ability.
  • Manage the allocation and compliance of team PPE (Personal Protective Equipment) to maintain safety standards.
  • Work using own initiative and maintain effective communications with People we Support, relatives and other members of the multidisciplinary/agency team.
  • Manage their team including reviewing and reporting absence, performing well‑being checks and providing support where available.
  • Manage rota inline with service needs and contractual obligations to staff.
  • Lead by example, show exemplary conduct and behaviour, support the team by reviewing concerning behaviour, and ensure it is recorded and dealt with appropriately.
  • Engage the teams in their area to deliver the vision and live the values, challenge behaviour at all levels that does not fit with the values, and demonstrate the organisation’s values in daily behaviour.
  • Lead and supervise junior staff who are providing care, act as a source of advice and mentorship, and facilitate ongoing development plans.
  • Report SSSC concerns immediately and document incidents and reports in a detailed and timely manner.
  • Refer to Health & Social Care Partnership following assessment of needs, including follow up on actions.
  • Perform risk assessments of staff when required, ensuring these are recorded and supplied.
  • Hear initial concerns of team and manage and resolve these appropriately, ensuring an appropriate solution is achieved.
  • Support by the HR department; participate in investigations, grievances and disciplinary actions, ensuring all relevant information and documents are completed timely and satisfactorily while upholding confidentiality.
  • Actively thank and recognise staff for their achievements, growth and demonstration of the values of the company.
  • Maintain a safe environment ensuring appropriate Moving & Handling is always used.
  • Undertake any other activities deemed necessary by the Service or Deputy Service Manager.
Safeguarding of Vulnerable Adults
  • Complete training on Adult Support & Protection of Vulnerable Adults and understand and implement ASP policies and procedures. Report any safeguarding concerns to the senior person on duty or to the Home Manager.
