Overview

An exciting opportunity has arisen for a Band 6 Youth Justice Specialist Nurse (22.5 hrs pw) working with the Youth Justice Service. You will work directly with young people to identify health needs to reduce their risk of offending. The YJ Specialist Nursing team provide health assessments to young people aged 10-18 who are working with the Youth Justice Service. Health appointments are delivered on an outreach basis and involve referrals to relevant mainstream and specialist health services, liaising with NHS and council colleagues. You will have regular contact with young people to build trust and deliver an individual package of care. A qualified SCPHN with significant experience in working directly with adolescents is required.

Responsibilities
  • Delivering outreach health appointments with young people across mid Cornwall area using youth justice CHAT assessments
  • Building trust with young people using a trauma-informed approach
  • Liaising closely with relevant professionals across health services, social care and youth justice service
  • Manage travel to various locations throughout the county, including areas that may not be accessible by public transport
Qualifications
  • Qualified SCPHN (Specialist Community Public Health Nurse)
  • Significant experience in a role working directly with adolescents
Additional information

This is a permanent post of 22.5 hours. The available post is based in the East area of the county and would require attendance at clinical appointments.
