A local government authority in London is seeking a Young People’s Pathway Manager to support homeless young people through the Routes to Independence Pathway. The role involves developing relationships with various stakeholders, managing referrals, and ensuring successful outcomes for youth transitioning to independence. Candidates should have significant experience working with vulnerable populations, excellent networking abilities, and strong time-management skills. This is a fixed-term position until September 2027 with a competitive salary range of £51,228 to £54,360.

The Supported Housing Commissioning Team commissions services for Lambeth people including homeless young people, young parents, children looked after, care leavers, unaccompanied asylum‑seeking young people, vulnerable adults, and individuals who are rough sleeping. This post will sit within this team but is an operational role focusing on the Routes to Independence Pathway for homeless young people, children looked after, and care leavers.
We invite you to join our lively, passionate, and welcoming team. You will play a key part in making a significant difference to the lives of the young people of Lambeth.
This operational role focuses on the Routes to Independence Pathway, which includes services that prevent homelessness, provide specialist housing‑related support, and enable successful move‑on. You will support young people through every stage of their journey toward independence and develop a strong understanding of Children’s Social Care placements within Semi‑Independent Living provisions to contribute to the overall success of the pathway.
As a front‑facing role, you will work closely with providers, commissioners, Children’s Social Care and Housing Teams, managing referrals into and out of the pathway, identifying barriers to move on and leading placement sustainment interventions to ensure the best possible outcomes for our young people.
• Support young people through the Routes to Independence Pathway stages and ensure successful outcomes.
• Develop and maintain effective recording systems and analyse data to inform decisions.
• Work with performance indicators and present complex issues to a variety of audiences in written or verbal formats.
• Network at all levels, building relationships to achieve objectives and improve processes and delivery.
• Manage multiple urgent tasks, prioritise workload and meet deadlines.
