A healthcare service provider in South Tyneside is seeking a Community Care Assistant to support customers discharged from the hospital. Responsibilities include providing care during transitions home, conducting assessments, and assisting with personal care. Candidates must have 2 years of home care experience and a valid driving license. We have an exciting opportunity for a HICCS Community Care Assistant to join an organisation making a real difference in people’s lives. We're looking for a committed, reliable and experienced care support worker to join our expanding Home Independence and Crisis Support Service (HICSS). This role is currently on a temporary basis to help with the increased demand for our services, but with the potential for the successful applicant to join our permanent team within the HICCS or domiciliary service. Currently the hours on offer are 8am to 8pm shifts.

Pay: £15.58 per hour
You’ll be supporting customers being discharged from hospital and returning home by ensuring the right care and support is provided during this period of transition including completing assessments, crisis interventions and emergency response, personal care, medication, supporting with use of daily living/rehabilitation aids. Applicants must have a full driving license and access to own vehicle as you will be expected to work across all areas of South Tyneside.
