A community health services provider is seeking a Virtual Ward Clinical Co-Ordinator in Grimsby. This role entails clinical coordination of patients to ensure they receive the appropriate care in their homes. Applicants should have at least 2 years' experience in primary or community care and work well in a multi-disciplinary team. The position offers competitive salary ranging from £36,806 to £44,717 annually, with a rota requiring weekend and holiday work. Full-time position with flexible working hours.
The Virtual Ward Teams are based withinNorth Lincolnshire and Goole Hospitals (NLaG) at both Grimsby and Scunthorpesites, to support patients from the hospital and community. The Teamco-ordinates the patient care and transition within a multi -disciplinary teamenvironment both in Acute and Community Care. This role will be pivotal inorganising and ensuring the right care is identified and delivered by the rightteams, so patients can remain looked after in their own home environmentsenabling better outcomes for patients wellbeing and recovery.

Based at Diana Princess of Wales Hospital, Grimsby, butmay be required to work at Scunthorpe General Hospital or any location withinNE Lincolnshire
37.5 hours per week, you will be required to work a rota pattern whichincludes weekends and Bank Holidays.

As part ofthe Virtual Ward Team, you have delegated responsibility for the clinicalco-ordination of people that have been stepped up or stepped down on to the VirtualWard which includes assessment of care needs, the development, implementationand evaluation of programmes of care and the setting of standards on a shiftbasis, ensuring delivery of quality care to people, who often have complexneeds in their own home, as alternative to acute bedded care. The role willparticipate in innovation and evidence-based nursing practice throughprofessional leadership and supporting the Virtual Ward clinical team in closeliaison with the Advanced Clinical Practitioner, and the Virtual Ward ClinicalLead. You are responsible for ensuring that staff on shift are regularlymonitoring and reviewing in-line with the care plan.
You will have a minimum of 2 years postregistration experience, working autonomously in primary or community care.
You will be experienced working as part of a multi-disciplinary team with multi agencies to optimise and promote the best care.
