Job description

Whitstable Medical Practice is looking for a Nurse/Paramedic Practitioner or Advanced Clinical Practitioner to join our Urgent Treatment Centre working 30 + hours per week. Applicants will need to have experience of urgent care, ambulatory care or same day emergency care.

We are looking for an experienced practitioner, who acts within their professional boundaries, to provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.

Main duties of the job

Applicants will demonstrate safe, clinical decision-making and expert care for patients within the general practice, the community and Urgent Treatment Centre. You will work collaboratively with the multi‑disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing clinical leadership as required.

We are looking for candidates who can work autonomously within our busy Urgent Treatment Centre. Excellent communication skills with evidence of leadership qualities and experience of using medical software package are also required.

Whitstable Medical Practice is a forward‑thinking single practice Primary Care Network (PCN) GP Practice based across 3 sites in Whitstable. There are 24 equity partners looking after over 45,000 patients. We pride ourselves on our innovative approach to Primary Care. We are a training practice and run various in‑house contracts including Cataract surgery, Ultrasound, Dermatology, Physical Therapies and Audiology together with further community contracts, a Day Surgery Suite and an Urgent Treatment Centre with digital x‑ray.

Responsibilities
  • Deliver a high standard of patient care within the community, Same Day Hub and Urgent Treatment Centre (UTC), utilising advanced autonomous clinical skills and a broad theoretical knowledge base.
  • Work confidently and independently without direct supervision.
  • Manage an autonomous clinical caseload, addressing the needs of patients presenting to the Same Day Hub and UTC with undifferentiated and undiagnosed conditions.
  • Manage own clinical workload, responding effectively to patient need while supporting patient choice and improving ease of access to services.
  • Develop and use appropriate referral pathways to local acute NHS Trust services, including Diagnostic and Treatment Centres.
  • Mentor and support colleagues and junior clinicians in developing and maintaining clinical skills.
  • Make professionally autonomous clinical decisions and accept accountability for those decisions.
  • Act as the first point of clinical contact for patients presenting with undifferentiated and undiagnosed conditions.
  • Use advanced skills in history taking, physical assessment, clinical reasoning and decision‑making to establish diagnoses and formulate management plans.
  • Initiate and interpret appropriate invasive and non‑invasive diagnostic investigations (e.g., blood tests, X‑rays).
  • If independent prescribing qualification is held, prescribe safe, appropriate and cost‑effective medication in line with current legislation.
  • Deliver evidence‑based, individualised, cost‑effective care aligned with best practice and clinical guidelines.
  • Provide holistic care for patients and families, developing ongoing care and support plans with an emphasis on prevention and self‑management.
  • Promote health and wellbeing through patient education, motivational interviewing, screening and health promotion activities.
  • Refer patients directly to appropriate services or agencies when required.
  • Contribute to the practice achieving quality and performance indicators to maintain high standards of patient care and service delivery.

  • Create and maintain a positive learning environment for patients, colleagues and students.
  • Contribute to the planning, delivery and evaluation of teaching for existing staff groups, including:

  • Promote evidence‑based practice using current clinical guidelines and contribute to practice‑based research where appropriate.
  • Monitor and evaluate the effectiveness of personal clinical practice through audit, peer review and other quality assurance strategies.
  • Maintain professional registration and adhere to requirements for continuous professional development.
  • Work within the most recent NMC Code of Professional Conduct (or relevant governing body).
  • Accurately and promptly record consultation data in patient records in line with NMC and organisational standards.
  • Stay informed about relevant health policies and work collaboratively with the practice team to implement necessary changes.
  • Work collaboratively and effectively with colleagues both within the practice and across external services.
Qualifications and Experience
  • Experience working in either Accident & Emergency / Minor Injuries or GP practice.
  • Nurse practitioner, Emergency care degree, relevant degree or equivalent level training in a related field. Experience in assessment, diagnosis and management of patients.
  • Emergency Nurse practitioner course, Accident & Emergency course or Enhanced Clinical Examination or Consultation Module, Minor injuries and Minor Illness course, Independent Nurse Prescribing, Primary Care experience, Mentorship and/or Leadership.
  • Experience of working either in an Accident & Emergency, Minor Injuries Unit or General Practice.
  • Taking history from a patient.
  • Understanding of local and national issues relating to urgent care.
  • Up to date knowledge of DH and NICE guidance.
  • Some knowledge in the management of minor illness/injury.
  • Fracture management / manipulation and simple plastering.
  • Suturing and wound closure (maintaining aseptic technique).
