Job description

Overview

The Urgent Community Response service is a key part of the integrated urgent care system for Lincolnshire by providing virtual and face to face assessment of patients in their own home experiencing an acute need that requires a 2 hour response.

An exciting opportunity has arisen to recruit to the existing multi-disciplinary team that is based across the county. The service runs 8-8, 365 days a year and there is a requirement to work a variety of shifts to cover opening hours, including weekend work. The successful post holder will be based at one of our bases (Lincoln, Boston, Spalding, Louth or Sleaford).

Responsibilities
  • To work as an accountable and autonomous practitioner within the Urgent Community Response team delivering patient-centred care, with emphasis on assessment of need, coordination of services, and planning to enable individuals to avoid admission to the acute setting, ambulance call out or conveyance.
  • To respond to people across Lincolnshire who require timely intervention and assessment of need and to liaise with other services to support patients to stay in their own home.
  • Independently assess patients referred into the service using specialist assessment to determine clinical diagnosis and treatment indicated, and maintain records as an autonomous registered therapist within a variety of clinical settings.
  • Use a wide range of skills and strategies to communicate with people about difficult matters or situations.
  • Manage a clinical caseload of patients with complex and specialised needs to a high professional standard and seek guidance from senior colleagues when necessary.
  • The role requires frequent moderate physical effort (e.g., moving and handling patients, mobile equipment, using hoists and other mechanical aids).
  • Car driving daily, varying distances in urban and rural communities, including during adverse weather conditions.
  • Flexible working opportunities to balance personal and service needs, including part-time, compressed hours, job shares and hybrid working models where arrangements meet service requirements.
