Tissue Viability Nurse - Leg Ulcer Specialist

We are looking for a Registered Nurse to join our lovely Nursing Team at Bridport Medical Centre. This role will be 18.75 hours a week, Monday, Wednesday and Friday.

Main duties of the job

To deliver a structured, evidence-based leg ulcer service inline with the Local Enhanced Service (LES) inLowerLimbUlceration. Managing patients within the general practice setting.This includes clinical assessment, Doppler/ABPI testing, application and reviewof compression therapy, patient education, and staff support using SystmOne.

About us

Ammonite Health Partnership is made up of 3 Practices. These are Tunnel Road Surgery in Beaminster, Pound Piece Surgery in Maiden Newton, and Bridport Medical Centre in Bridport.

Our current team includes: 7 GP partners, 4 Salaried GP, 6 Advanced Clinical Practitioners, 1 Visiting Paramedic, PCN pharmacy team, PCN social prescribing team, with comprehensive PCN based frailty and community care cover.

A GP led triage hub contributes to workload management, to reduce workload and maximise clinically relevant face to face consultations. An experienced workflow team minimises document processing with pharmacist input for discharge and other medication management.

Ammonite Health Partnership is a member of Jurassic PCN, covering approximately 64% of the PCN patient population.

Job responsibilities

Key Responsibilities

Clinical Care

  • Conduct timely leg ulcer assessments following the Lower Limb Ulceration specification.
  • Perform Doppler/ABPI assessments to guide compression decisions.
  • Deliver and monitor appropriate compression therapy in line with best practice.
  • Accurately document all care using SystmOne leg ulcer templates.

System Use SystmOne

  • Maintain clear, timely and pathway-compliant documentation.
  • Record Doppler results, ulcer healing progress, and wound photographs (if used).
  • Ensure correct use of templates for audit and reporting purposes.

Education & Team Support

  • Support and supervise HCAs and Practice Nurses in compression and wound care.
  • Run short in-practice training sessions or updates as needed.
  • Promote adherence to the Dorset Pathway across the practice team.

Service Improvement & Governance

  • Monitor patient outcomes (e.g. healing times, compression compliance).
  • Participate in wound care audits, service reviews and QI projects.
  • Maintain safe, effective, and patient-centred leg ulcer care within the practice setting.

Experience
  • Wound care or tissue viability experience (ideally leg ulcer-focused)
  • Skilled in Doppler/ABPI and compression bandaging
  • Competent in using SystmOne, including wound care templates
  • Familiar with the Dorset Leg Ulcer Pathway
  • Experience of infection prevention and control measures
