Overview

As a Team Leader at Avery Healthcare, your focus will be to support the home management team in delivering high-quality, person-centred care. You will be responsible for implementing, monitoring, and evaluating care plans, assisting with smooth transitions of resident discharges, and leading in the monitoring of resident well-being. You will also be expected to practice safe systems of work and demonstrate the ability to lead staff effectively.

  • Lead in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assist in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed where appropriate.
  • Lead with monitoring the well-being of each resident, be aware of any change impacting care and care planning, and ensure that changing needs are met and written records are amended to reflect any variation.
  • Practice safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
