About the role

RIACT is colocated with health colleagues across two sites. The aim to support individuals who are acutely unwell to regain their independence, always promoting a home first approach. They work alongside colleagues to focus on individuals strengths, utilising services around them to create a safe environment to progress.
To complete facilitate hospital discharges and review following period of recovery.
To complete reablement assessments, CSNA, MCA, risk assessments, Saf safeguarding, CHC and DST's.

If you have experience within Adult Social Work and want to be part of a forward thinking, fast pace and supportive team get in touch.

  • Registered with Social Care Wales/ Social Work England
