Responsibilities

• Conduct specialist Occupational Therapy assessments for patients.
• Set and evaluate rehabilitation goals for clients.
• Evaluate therapy progress and adapt interventions as needed.
• Perform risk assessments for complex patients.
• Communicate effectively with patients facing various challenges.
• Conduct home visits to evaluate discharge risks.
• Offer support and education to patients and their caregivers.
• Liaise with external agencies for discharge planning.
• Maintain high standards of practice with senior therapist support.
