Overview

Job Title: Staff Nurse, Hospice at Home and Day Hospice

Band: Band 5 (Agenda for Change) - GBP31,049 - GBP37,796 pro rata, depending on experience

Hours: Various hours - part time or full time (up to 37.5 hours per week)

Responsible to: Hospice at Home/Day Hospice Team Leader

Accountable to: Head of Clinical and Operational Services

Department Base: The Shakespeare Hospice, Shottery, Stratford upon Avon CV37 9UL

We have an exciting opportunity for a part time staff nurse to work within The Shakespeare Hospice Adult Community Services.

Responsibilities
  • Be a core member of the multidisciplinary palliative care team.
  • Work across Hospice at Home and Day Hospice services covering South Warwickshire.
  • Collaborate closely with other Hospice and community teams to promote holistic, person-centred care for patients, their families and those requiring specialist palliative support.
  • Contribute to the provision of care for patients with life-limiting illnesses, including carer and bereavement support.
  • Uphold hospice values: compassionate, accessible, respectful and effective; maintain excellent standards and contribute to future service development in partnership with Team Leaders.
