Job description
Overview

Liverpool Diabetes Partnership is a community-based service supporting people living with diabetes across Liverpool. As a Band 6 Community based Specialist Diabetes Dietitian you will deliver dietary education, develop individualised care plans and work across community clinics, domiciliary visits, and virtual and face‑to‑face settings. You will support the DAFNE programme, collaborate with an Advanced Diabetes Dietitian and the multidisciplinary team, supervise student and graduate dietitians, and contribute to service development and quality improvement.

Responsibilities
  • Provide dietary education and support to patients with Type 1 and Type 2 diabetes using digital, written, and face‑to‑face resources.
  • Contribute to MDT diabetes clinics across Liverpool in partnership with specialist nurses and consultants.
  • Deliver education and support to nursing and care homes throughout the city.
  • Leverage new diabetes technologies, including injectable therapies, real‑time blood glucose analytics and insulin pumps.
  • Participate in audit, key performance indicators and service development initiatives.
  • Engage local communities to promote equitable access to care and develop culturally relevant resources and peer support.
  • Manage and prioritise your own workload and communicate effectively with service users, carers and agencies.
Qualifications
  • Leadership and communication skills to supervise students and graduate dietitians.
  • Strong NHS multi‑disciplinary teamwork experience.
  • Computer literacy and familiarity with electronic health record systems.
  • Ability to articulate the case for nutritional intervention within care plans.
  • Excellent presentation and communication abilities.
  • Operational service‑delivery experience.
  • Advanced knowledge of a defined area of clinical practice and current best practice.
  • Evidence of continued professional development and specialist knowledge in dietetics.
  • Current registration with the Health and Care Professions Council (HCPC).
  • Membership of the British Dietetic Association (BDA) or equivalent professional indemnity scheme.
Desirable Criteria
  • Introductory level Clinical Supervisory Skills Training.
  • Broad clinical experience in at least three clinical areas, including medical and surgical patients.
  • Experience of multi‑disciplinary working.
  • Demonstrated ongoing professional development.
  • Experience training student dietitians.
  • Experience conducting audit.
  • Advanced IT skills (PowerPoint, e‑mail, desktop applications).
