A leading healthcare provider in Carmarthen seeks dedicated rotational pharmacists to join their dynamic pharmacy team. This role involves working across multiple wards, providing clinical services, and contributing to patient-centered care. Candidates should possess a master's in pharmacy and be accountable for enhancing medication use. The position offers a salary range of £48,527 to £55,532 per annum, reflecting the importance of skilled pharmacy practice.
We are excited to offer an opportunity for two experienced and motivated rotational Pharmacists to join our dynamic Pharmacy team at Glangwili General Hospital. You will be working across a variety of general medical and surgical wards, within the Emergency Department and the Women and Children's wards, delivering a proactive, patient-centred clinical pharmacy service. You will be key members of the multidisciplinary team, contributing to safe, effective and evidence-based use of medicines to improve patient outcomes. You will deliver a high quality, patient focused clinical pharmacy service by carrying out comprehensive medicines reconciliation, provide expert advice on the safe, effective and appropriate use of medicines, assess prescriptions for clinical appropriateness and support prescribers in complex decision-making, supply medication in a timely manner. You will support the Lead Pharmacist by contributing to the development and implementation of guidelines and protocols, participate in clinical audit, incident reviews and quality improvement projects to enhance patient care.
