A local authority council in Wales is looking for a dedicated individual to assess adults for mental health services. Responsibilities include promoting independence and providing information to clients. Ideal candidates will have experience with vulnerable adults and a recognized degree in Social Work. This role offers a competitive pro-rata salary and is located at Llanfair Unit, Llandough Hospital.
The Vale of Glamorgan Community Mental Health Service for Older People is an integrated team made up of local authority and health professionals with specialist knowledge of mental disorders. Professionals within our team include Social Workers, Approved Mental Health Professionals, Psychologists, and Psychiatric nurses, Occupational Therapists, Physiotherapists and Psychiatrists. We hold a weekly meeting that affords time to discuss complex cases in a multidisciplinary arena.
Our service offers secondary mental health care for adults living with dementia and people over 65 years diagnosed with mental ill health.
Through the provision of recovery focussed care and treatment, we encourage individuals to adopt a strengths based approach and offer support in achieving personal outcomes. Our team work closely with individuals and families to ensure they are safe and live well in their own communities.
Pay Details: Grade 8 Scp 26-30 £37,280‑£40,777 Pro Rata £19.32‑£21.14
Hours of Work: 37 hours
Working Pattern: Mon‑Fri
Main Place of Work: Llanfair Unit, Llandough Hospital
To assess adults to determine eligibility for the provision of services, promote user empowerment and independence. To offer appropriate information advice and signposting to alternative services to meet identified needs.
