Job description
Job Overview

Your main responsibility will be to focus on the people you care for and what is right for them. You'll cover the whole of Central Bedfordshire and out-of-area hospitals, and manage your own caseload, which will include supporting a set number of people through discharge pathways (from hospital and discharge to assess). You will also work closely with the reablement team, who are the first port of call out of hospital. Our aim is to support people at the most vulnerable point in their lives to get home and recover safely, consequently becoming as independent as possible. This is a very diverse role where every day is different and presents a new challenge. Some of the areas of work you can expect to be involved in are:

  • Duty function
  • Discharge to assess (going out into the community - visiting residents in their homes, care homes, hospital, etc.).
  • Home recovery service
  • Care act assessments - working within legislation and latest hospital discharge guidance.
  • Mental capacity act assessments.
  • Discharge planning.

Positions Available

We are looking for experienced Registered Social Workers, Nurses or Occupational Therapists to join our professional and dedicated Hospital Discharge team. We currently have available 2 full time positions. We provide a 7-day service, operating in a hybrid working pattern with rest days in the week. Our goal is to create a strong network of supportive individuals who thrive in a fast-paced environment. This role will suit someone who is eager to get things done and make a real difference in their community. The role will involve going into hospitals, resident's homes, nursing homes and residential care homes to maximise people's independence, with a strong focus on improving their futures. We need people to be flexible, have experience of working in these environments, and approach every day with a "can do" attitude.
