A local authority in the UK is seeking a passionate Social Worker to join their Advice and Wellbeing Hub. As a Social Worker in the Advice and Wellbeing Hub, you will be the first point of contact for adults, carers, and professionals seeking advice and support. You’ll provide strengths-based assessments, offer guidance on available services, and ensure people receive the right interventions to promote independence and wellbeing.
The Advice and Wellbeing Hub is a busy, supportive and friendly team, working with people of all ages and backgrounds and their families and carers. We use a Conversations Count strengths-based model to promote choice and control and achieve excellent outcomes.
You will work with individuals who have been referred for support for up to 6 weeks, to maximise their independence in their own homes. Promoting choice and control and using a strength-based approach.
The role involves you undertaking work using the Conversation Counts model, working on prevention and early support. Using your social work expertise, you will conduct Care Act assessments, safeguarding investigations, and Mental Capacity assessments, as well as facilitate best interest meetings. You will also manage a varied caseload, ensuring individuals receive the right support to maintain their independence.
This role involves working with adults with diverse needs, including individuals with learning disabilities, physical disabilities, older adults, and those affected by substance or alcohol misuse. Additionally, you will provide information, advice, and support to carers, helping them navigate available resources and services.
