Join our dynamic team based at the Adults with Learning Disabilities Team, Town Hall & Civic Offices, Westoe Road, South Shields, where you will be part of transformative change, working full or part-time to empower individuals and help them live better lives.

Location: South Shields, Tyne and Wear, England – NE34

Responsibilities
  • Conduct person-centred assessments and empower individuals to make informed choices in their care.
  • Collaborate with individuals who have complex needs to create support plans that enhance their abilities and help achieve their goals.
  • Work jointly with different agencies and professionals to ensure the holistic needs of individuals are met.
  • Manage a diverse caseload while safeguarding individuals, maintaining practice quality, and ensuring service compliance.
  • Promote independence by embedding strength-based approaches in practice and support planning.
  • Participate in and contribute to the development of Adult and Integrated Care Services, adapting to local and national policy landscapes.
  • Safeguard vulnerable adults, conducting investigations in line with South Tyneside safeguarding procedures.
  • Provide support and guidance to less experienced staff, fostering an environment of learning and development.
  • Maintain accurate records and high data quality standards.
