What you will do:

  • Work within a fast‑paced team to provide support to adults in the community dealing with Autism, Acquired Brain Injury (ABI), and physical health conditions.
  • Collaborate with families and other professionals to develop and implement comprehensive care plans.
  • Manage and support the duty system and ensure all safeguarding measures are in place.
  • Utilise your expertise in COPDOL and safeguarding to navigate complex cases efficiently.
  • Regularly assess client progress and adapt care plans as necessary to meet changing needs.
  • Communicate effectively and build meaningful relationships with clients to understand their personal needs and goals.
