What you will do:

  • Conduct comprehensive assessments of clients’ health and social care needs, ensuring a high quality of service
  • Initiate and coordinate robust support plans that promote independence while taking advantage of community strengths
  • Identify opportunities for Reablement and enablement and connect clients with these services when needed
  • Foster independence and wellbeing using a Strength-Based Approach, encouraging clients to assess their needs and risks
  • Maximise use of community resources and preventative services to better meet individuals’ goals
  • Monitor and review service provision with a sharp focus on delivering positive outcomes
  • Collaborate with local providers to enhance service quality and effectiveness
  • Provide detailed advice, information, and support to clients, carers, colleagues, and partner agencies
  • Engage in safeguarding activities and work as a Safeguarding Officer, ensuring the protection and well-being of vulnerable adults
