What you will do:

  • Conduct strengths-based, person-centred assessments under the Care Act 2014, identifying social care needs and housing vulnerabilities.
  • Develop, implement, and review support plans, including carers’ assessments and tenancy sustainment strategies to ensure holistic care.
  • Work to safeguard adults, including conducting mental capacity assessments and making Best Interest Decisions where necessary.
  • Provide clients with access to housing, welfare benefits, mental health, and community-based resources to promote independence and well-being.
  • Collaborate across a multi-agency landscape—including NHS, housing officers, charities, and probation services—to secure positive outcomes for service users.
