Social Worker – Adult Assessment or Safeguarding Team
Sanctuary Personnel has a new permanent position for a Social Worker within the Adult Assessment or Safeguarding Team to work Full Time based in Haringey. The salary is up to £51,574 per annum.
Responsibilities
- Take responsibility for a suitable caseload and develop experience and competency across duties with opportunity to progress from one grade to the next when progression requirements are met.
- Develop and maintain effective relationships with service users, families, carers and communities to help them make informed decisions, clarify and express their needs, and contribute to person-centred, strengths-based service planning.
- Work effectively within legal, policy and financial frameworks, obligations, structures and culture, particularly the Care Act, Mental Capacity Act, Mental Health Act, Human Rights and Equalities legislation and accompanying guidance and codes of practice.
- Construct and implement care packages linked to client needs, working with community and voluntary organisations to maximise independence, minimise risks, and improve outcomes and quality of life for service users and their carers.
- Be responsible for the assessment and safe discharge of service users from hospital back into community settings, within challenging timeframes and reducing the risk of future re-admission.
- Undertake a range of Care Act Assessments and Section 42 enquiries through the safeguarding process, identifying risks, needs and options, including self-directed support options.
- Undertake Mental Capacity assessments for service users when capacity to make a specific decision may be in question.
- Identify service users eligible for Continuing Health Care (CHC) or Section 117 funding and undertake checklists; work with other professionals to provide a multi-disciplinary assessment within this framework.
- Complete Human Rights Act assessments for those who have no recourse to public funds but may require support under the Care Act.
- Manage an individual caseload of clients, regularly monitor and review changes in needs, and ensure services contribute effectively to the support/care plan and resource use.
- Conduct ongoing and urgent case reviews as necessary.
- Intervene in emergency situations to protect vulnerable adults and children and initiate appropriate statutory or other actions.
- Participate in office duties on a rota basis, respond to new referrals, assess urgency, and implement actions to meet immediate needs.
- Maintain and update case notes and records within required timescales, write reports, provide written responses as required, and give evidence in court in relation to care or other proceedings.
- Develop and maintain up-to-date knowledge and competency in relevant legislation, social work practice, and changes in social work delivery models.
- Work effectively as a member of a team and contribute to team meetings, forums and organisational events to the development of the service.
Requirements
- A Social Work Qualification or equivalent.
- Understanding of relevant legislation.
- Registered or eligible for registration.
