Role – Social Care Worker (Health and Social Care Connect)
Location – GCHSCP, 100 Borron Street, Glasgow, G49XG
Contract Status – Permanent

Glasgow City Health and Social Care Partnership (GCHSCP) is on a journey to radically change the way in which Social Work delivers support and services.

An exciting opportunity has arisen for Social Care Worker posts within the Health and Social Care Connect (HSCC) Service.

HSCC service is a single point of access for Social Work Services within GCHSCP. Changing the way we deliver services using a Strength Based approach and have effective interface with partners and the 3rd sector. The service will span across Adults and Older People’s Services, Children and Families and Homelessness, working in a multi‑disciplinary way.

We are seeking energetic, talented, resilient and committed social care workers to prevent long term involvement with statutory services where possible and provide excellent services to those who require it using an early intervention and prevention approach whilst maximising independence.

You will have sound knowledge of assessment and care case management; have practised under the relevant legislation; and work in a person‑centred manner which will be fully embedded in your practice. You will be confident in managing risk and looking for innovative ways to promote independence and link people into the local community.

Applications will be considered from people with experience from the aforementioned care groups, including occupational therapy assistant experience.

HNC/ SVQ3 in Social Care or equivalent qualification would be advantageous. Please refer to the Person Specification for the required qualifications, skills, knowledge and competencies that are required for these posts.

Membership of the Protection of Vulnerable Groups (PVG) Scheme is essential on taking up appointment. Please note successful candidates for the above role will be required to pay for their PVG Membership/or Scheme Checks.

These roles are excepted under Rehabilitation of Offenders Act 1974 (Exclusions and Exceptions) (Scotland) Order 2013 (as amended).

Interviews are expected to take place week commencing 12th January 2026 and invites will be sent out the 5th January.
