Salary: £13.26 per hour
Job purpose: To provide personal care, emotional and practical support to people with identified care and support needs within a Registered Care Home setting. This includes older people, people with a diagnosis of dementia and/or adults with a learning or physical disability. The role supports a homely atmosphere while promoting the rights, dignity, diversity, independence and wellbeing of individuals, with flexibility to work additional hours across other homes.
Main responsibilities
- To implement and sustain the principles of a “meaningful day” by supporting individuals to participate in activities that give direction and purpose to their daily lives.
- To offer meaningful activities that address individuals’ physical, sensory, cognitive, emotional and social needs and capabilities, based on each person’s Life History Profile, with proactive support, advocacy, monitoring and evaluation of activities.
- To support and enable individuals to undertake all tasks of daily living skills (e.g., washing, dressing, administration of medication), while promoting ability and independence as documented in the Personal Plan.
- To support individuals to access local and wider community activities, including medical or other appointments.
- To perform duties of the Care Home in full compliance with the Regulation and Inspection of Social Care (Wales) Act 2016 and with service and corporate policies and procedures.
- To contribute to a range of services to meet the needs of individuals and participate in quality assurance systems in the service.
- To support individuals and representatives to contribute to, monitor and review personal plans and to help individuals achieve personal outcomes identified within them.
- To contribute to care management and inter-agency reviews of individual needs.
- To evaluate the risk of abuse, failure to protect and harm to self and others, and to promote safeguarding responsibilities in line with Wales Safeguarding Procedures.
- To participate in supervision and professional development requirements and maintain Registration with Social Care Wales.
- To maintain continuous professional development and identify training requirements, participating in training aligned with departmental strategies.
- To contribute to the maintenance and safety of the building and its contents and to undertake general domestic duties within the Registered Care Home as scheduled.
- To liaise with and work effectively with a wide range of multi-agency colleagues and support networks, internally and externally.
- To demonstrate willingness and ability to work within any Registered Care Home within the Directorate.
- To undertake any other duties as directed by the Director of Social Services that are legal and reasonable at the time of instruction.
Formal qualifications
- Essential
- Completion of a 10 hour online Welsh Work course.
Technical skills required
- QCF Level 2 in Care or a similar qualification recognized by Social Care Wales, or willingness to undertake this within the first year of employment.
- Competent in the following topics or able to complete training: Safeguarding; Care with medicines; Control of infection; Food safety; Moving & handling; Fire safety; Sensory impairment; Mental health awareness; Continence care; Care & responsibility; First aid; Commitment to undertake training relevant to supported individuals.
