A local government authority in England seeks a Social Care Practitioner to join their Transition team. This role involves supporting vulnerable adults to promote their independence and wellbeing. About the Role
You will collaborate with individuals, carers, agencies, and support brokers to support vulnerable adults and their families, promoting independence and choice. You will manage a caseload with complex needs, undertake safeguarding enquiries, and conduct community assessments.
You will work in a team with professional supervision and supportive colleagues.

Applicants must have a valid UK driving licence and access to a vehicle, with willingness to travel. Reasonable adjustments will be made for disabilities.
The current contractual location is Quadrant Court, with plans to move to Victoria Gate, Woking later this year.
