Job summary

A local authority in Hull is seeking a passionate Social Care Advisor for a fixed term of 12 months. This role involves acting as the first point of contact in a multi-disciplinary team, providing support and interventions for vulnerable adults. Ideal candidates will have experience in social care settings and knowledge of relevant legislation. The position requires flexibility for offsite assessments. Social Care Advisor Grade 6 – fixed term 12 months

Are you passionate about making a positive and lasting difference for the people of Hull? Care and support in Hull is transforming and Hull City Council are looking for exceptional individuals like you to join the Adult Social Care team. If you have experience of working with vulnerable adults and in a service area as a first point of contact, along with knowledge of relevant social care legislation, policies and guidance then we have an exciting opportunity for you.

You will work as a first point of contact within a multi‑disciplinary team in the delivery of meaningful, person centred and solution focused interventions which identify the person as the expert in their life. This includes the new Integrated discharge Team working and our new Discharge to Assess model. Your conversations will enable people to shape their own lives, identify their own outcomes and steps forward and provide interventions to sustain their independence.

You will use the online information, information gathered from the person, ward and signpost where appropriate to maximise peoples independence and support individuals home/discharge from hospital.

Please note that 7 day working including bank holidays will be required and it is anticipated this will not be more than 1 weekend in 3. This post will be supporting through our discharge to asses model and we have different office spaces and the need for flexible individuals who may be needed to go to different environments to support assessments, e.g other care units and the community including peoples own homes. Further details will be discussed with you once in post.
