Job description

Overview

You will be working as part of the Rapid Response Team predominantly on Unplanned Care to triage and assess referrals into the service; You will be accountable for initial history taking, clinical assessment, triage, prescribing (NMP qualification is essential for this role), and onward referral. There is an expectation that with demand in planned care you would be flexible to cross cover.

Responsibilities

• Undertake a full holistic assessment during face-to-face patient visits, plan care with the patient at the centre, and undertake physical examination of patients with undifferentiated conditions.
• Work closely with other members of the MDT and partners to formulate a management plan that supports the patient to remain in the community.
• Deputise for the Primary Care at Home or Service Manager as required to ensure Unplanned Care pathways are delivered to a high standard.

Requirements

• Enthusiastic and motivated nurse with community experience, who is passionate about delivering high quality care.
• Excellent communication, time management skills and sound clinical knowledge; a flexible approach to accommodate changing needs and contribute to service development.
• Prescribing experience with NMP qualification; driving license and access to a vehicle to carry out duties.
• Ability to work a shift pattern covering 7 days a week from 08:00–20:00.
