Senior Community Nurse

An exciting opportunity has arisen for a Senior Community Nurse to join the Neighbourhood Teams in Pershore and Upton.

We are looking for applicants with a passion for community nursing and delivering a high standard of care for patients. This role involves being a role model for junior members of the team and managing with a compassionate leadership style and a positive outlook to drive continual service improvement.

The successful applicant will have the ability to manage caseloads with evidence of strong clinical skills and decision making. This role will give you the rewarding opportunity to provide optimal care to patients in their own home and to develop your career to the next step. Shift patterns are between 07:30-18:30.

Due to the nature of the posts the applicant must be able to travel independently / have access to a suitable vehicle for business purposes and hold a full UK driving license.

These are part time posts working 30 hours per week.

Main duties of the job

Working flexibly to provide the service delivery need across the local area (the service operates from 7:30 to 18:30, Monday to Sunday, including bank holidays).

Your typical working day will include home visits for adults over 18 years needing a range of nursing interventions. These include leading on caring for people reaching the end of their life, managing their pain relief and other symptoms in a sensitive and compassionate manner, all types of wound care – post operative wounds, leg ulcers, pressure ulcers and complex dressing regimes and urgent care response work. You will undertake holistic assessment of a person’s needs, taking into account their environment and social circumstances.

Back at your base, you will lead team meetings, goal/care planning and evaluation and handover meetings. You will also support the teaching and learning of nurse students pre and post registration. You will oversee patients and assist in decision making regarding their nursing care.

Qualifications
  • Registered Nurse (RN1)
  • Specialist practitioner District Nursing (degree / diploma) or equivalent level nursing qualification
  • Non-medical prescriber (V150) or will be required to undertake this qualification on commencement in post
  • Evidence of on-going continuing professional development
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
