A community health service in Ashford seeks a dedicated support worker to provide high-quality, person-centred care. Are you ready to start a new career pathway working within a joint enterprise of Health and Social Care? You could become part of this combined service bringing the wrap around recovery service from hospital discharge. The principle of the service is recovery aimed at helping the person become as independent as possible through enablement.
This is a newly developed post working across Health and Social Care, supporting recovery and delivering enablement in all areas of daily life including personal care and supporting nutrition, whilst also monitoring the patient’s condition. You will contribute to an overall plan of care, including the support of the multi-disciplinary teams from Health and Social Care. You will play a pivotal role in delivering high-quality, person-centred care within the Home First service, providing advanced assessment, complex case management and operational support, while guiding and developing colleagues to maintain excellence across the team.
