About this client/child: Jake is a cheeky, energetic, and outgoing young chap with a love for swimming, train rides, and sensory play! He enjoys playing in his sensory room and in general creating some mischief! Jake lives at home in Bray, Berkshire with his family and has a care team who provide day and night support. He has Autism Spectrum Condition (ASC), a Learning Disability, Development Delay, and a Visual Impairment. When at home Jake is mobile, and when outside or for longer journeys he uses his wheelchair. Jake strives for his independence, and his family and team support and assist him to reach all of his targets and goals.
Jake requires 1:1 assistance and support including assistance with personal care, self‑help skills, supporting with activities and outings, preparing meals and snacks, assisting and implementing therapy programmes (training provided by Jake s therapist), and arranging fun and engaging learning and play opportunities. Jake wants to make the most of his weekends, so the more activities planned the better! The role will involve supporting Jake with self‑regulation and helping him to manage his expressive behaviours. It is essential that you are confident with documenting and recording accurate information, as Jake has a care log to be completed daily. Additional hours are available if desired, especially during school holidays.
A candidate with excellent ASC experience, confident, a strong communicator and dedicated. You will bring new ideas, be creative and motivational. Jake s requires an SEN Support Worker that will encourage and support him to reach is potential. Due to the family home location, you must be a driver with your own transport.
Supporting Jake is the best part of this role, he is a funny, gregarious young chap with a zest for life! You will be working as part of a dedicated, organised and professional team, and will receive regular training and supervision by Jake s case management team.
