About the Role

As a Resettlement Broker, you'll be part of a pioneering service commissioned by Oxleas NHS Trust, working closely with healthcare teams to support service users both in custody and through the gate into the community. You'll play a vital role in developing and delivering tailored resettlement plans, offering hands-on support from group sessions to community integration.

Responsibilities
  • Engage with healthcare referral agencies to identify and support service users.
  • Conduct assessments and design personalised support plans.
  • Deliver in-custody group sessions and provide community-based support post-release.
  • Collaborate with external agencies to ensure continuity of care.
  • Maintain accurate records and contribute to service development.
  • Recruit, train, and manage volunteers and peer mentors.
