We are looking for a Registered Nurse to join our stroke service on the Hyper‑Acute Stroke Unit/Ward 23 at York Hospital.
Over the past 20 years stroke care has changed dramatically. Stroke is now treated as a medical emergency, where time is brain, and our service has developed to reflect this.
Our service comprises a dynamic multidisciplinary team that delivers evidence‑based patient‑centred care along our stroke pathway, from admission through to discharge to the community.
Patients are admitted directly via the emergency department, GP referral or inpatient areas to our 26‑bedded Hyper‑Acute Stroke Unit. Band 6 Hyper‑Acute Stroke Nurses work closely with the Band 5 Registered nursing team to provide a nurse‑led 24/7 clinical assessment service for thrombolysis and thrombectomy for individuals across the populations of York, Scarborough and a proportion of Harrogate. As a valued member of the ward team the registered nurse you will play an integral part in the patient journey, aligned to the National Stroke Guidelines.
Our stroke service is measured nationally by the Sentinel Stroke National Audit Programme and works with a multiprofessional focus on continuous improvement.
The interviews for this position will be held in person at the York Hospital site with no remote interview option available.
Registered nurses are responsible for direct patient care, adhering to their professional standards as outlined by the NMC.
Assessment of patient care needs, implementation of individualised care planning and subsequent evaluation of planned care. Updating of patient‑specific risk assessments using the trust electronic recording systems, ensuring accurate, contemporaneous documentation standards are followed.
You will be responsible for the supervision and direction of junior staff, health‑care support workers and delegation of appropriate tasks for direct patient care.
They will be expected to support student nurses with their learning and act as assessor/supervisor during their ward‑based learning.
They will maintain effective communication with all members of the multidisciplinary team attending board rounds, liaising with the team to provide patient‑centred care and interventions. Effective and empathetic communication with patients and relatives ensures accurate, compassionate information is shared appropriately and in a timely manner.
The individual will be expected to work within the trust standard of behaviours/trust values and maintain healthy working relationships within the ward team.
