Overview

Qualified Social Worker, Team Leader, Hospital Discharge
Pay rate to £40 per hour
Contract role

We are recruiting for an experienced Social Worker to work in a Hospital Discharge Team in Halifax Council. A team leader is needed to cover maternity leave until August 2026. The team lead role is within the hospital discharge and post discharge assessment team. Approved for 30 hours a week. Hybrid – 3 days onsite. We cover Calderdale Royal Hospital and Huddersfield Royal infirmary. Must be able to attend these bases as well as Princess Buildings in Halifax.

Responsibilities
  • Lead in hospital discharge and post-discharge assessment within the Hospital Discharge Team.
  • Cover maternity leave until August 2026; week- by- week responsibilities on the team.
  • Work on a 30-hour per week contract with hybrid working (3 days onsite).
  • Attend bases at Calderdale Royal Hospital, Huddersfield Royal Infirmary, and Princess Buildings in Halifax as required.
