An exciting job role has recently become available at Luton Borough Council. They are looking for a new member for their Discharge, Assessment and Rehabilitation Team.

Responsibilities:

  • To Conduct comprehensive assessments of adults in need of social care intervention, taking into account their psychological, cultural, and emotional needs.
  • Collaborate with healthcare professionals to develop effective discharge plans. Ensure that service users are safely discharged and have the necessary support in place.
  • Provide support and guidance to service users to help them regain their independence through rehabilitation.
  • Manage a caseload of service users, regularly reviewing and updating care plans as necessary
  • Work as part of a multi-disciplinary team, maintaining effective communication and cooperation with colleagues from various disciplines.

What we are looking for in a Social Worker:

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
