You will coordinate services and resources to ensure smooth transitions for patients leaving the hospital. Additionally, you will demonstrate expertise in adult social care legislation and practices.

Qualifications and Experiences:

To be eligible for this role, you must have Social Work England registration and be eligible to work in the UK. Additionally, you need to hold a full UK driving licence and possess post-qualified experience. A degree or equivalent qualification in Social Work is also required.
