An exciting job role has recently become available in Islington. We are looking for a new member for the Hospital Discharge Team.
You will conduct thorough assessments under the Care Act to determine patients’ needs and support requirements. You will coordinate services and resources to ensure smooth transitions for patients leaving the hospital. Additionally, you will demonstrate expertise in adult social care legislation and practices.
To be eligible for this role, you must have Social Work England registration and be eligible to work in the UK. Additionally, you need to hold a full UK driving licence and possess post-qualified experience. A degree or equivalent qualification in Social Work is also required.
