Job description

We are seeking a qualified and accredited Psychological Wellbeing Practitioner (PWP) to join an integrated primary care mental health team at HMP Full Sutton. You will provide high-volume, low-intensity psychological interventions to individuals experiencing mild to moderate anxiety and depression, in line with Step 2 IAPT guidelines. Interventions will be delivered in both 1:1 and group formats, using CBT-based self-management approaches. You will work as part of a wider integrated team, supporting individuals throughout their time in custody and contributing to onward care planning for release into the community.

Key Responsibilities
  • Accept and triage referrals in line with agreed service protocols
  • Assess suitability of referrals and step individuals up or down the care pathway as appropriate
  • Conduct person-centred assessments, including risk assessment to self and others
  • Deliver low-intensity, evidence-based interventions including:
    • Guided Self-Help
    • Psychoeducational interventions
    • CBT-informed self-management approaches
  • Support individuals to self-manage their mental health and recovery
  • Record and monitor clinical outcomes using validated outcome measures
  • Maintain accurate, timely clinical records on agreed electronic systems
  • Attend and contribute to multidisciplinary team meetings
  • Work collaboratively with colleagues to ensure effective stepped‑care delivery
  • Liaise with statutory and non‑statutory services to support continuity of care on release
  • Contribute to performance monitoring and monthly data collection
  • Uphold confidentiality, safeguarding, and information governance standards
  • Act in accordance with organisational values and professional standards at all times
