We are seeking an experienced and dynamic Programme Manager to lead the development and implementation of the Southwest Frailty Learning Hub. This is a unique opportunity to shape a regional resource that will enhance frailty awareness, improve workforce capability, and support better outcomes for individuals living with frailty across the Southwest.
We are an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital‑based care to improve people's physical and mental health and also provide specialist support to people with dementia a learning disability. Over 4,000 people make up the Trust, including doctors, nurses, therapists, plus admin and support staff. We work in people's homes, in community clinics and bases, and some staff work from one of our 13 community hospitals.
