Overview
Professional Development Nurse in End of Life Care. To support the development of enhanced clinical skill in end-of-life care, ensuring access to appropriate development to maintain a competent workforce. The post holder will provide mentorship and work-based learning facilitation, in-house curriculum design and teaching in end-of-life care. To perform an expert clinical advisor and leadership role and professional support for community teams, to ensure safe and high-quality, evidence-based care delivered efficiently and cost-effectively, with performance management targets met and evidenced through data production and analysis. Develop clinical governance through the support, monitoring and implementation of enhanced clinical standards and learning within the clinical services.
Main duties
- The post holder is an autonomous practitioner and will assess, plan, implement and evaluate patient care and also participate in the training of patients, relatives, carers, students and other health care professionals.
- You will be committed to the development of the team through effective leadership and be given the opportunity to develop your own clinical skills through the delivery of a wide range of evidence-based care and an active personal development plan. You must possess excellent communication, assessment and care planning skills and have an excellent knowledge of clinical standards.
- Proven Senior Nurse clinical experience at Band 6 or equivalent in an acute or community setting, minimum two years working at this level.
- Community Nursing experience
- Experience of effectively working in collaboration and negotiating across organisational boundaries.
- Experience of change management and implementing in practice
- Teaching/mentoring experience
- Experience of receiving and delivering clinical supervision
- Ability to undertake audit.

Job responsibilities
Clinical
- To act as a role model for clinical staff, addressing patient needs and ensuring the delivery of safe patient care.
- Keep accurate, contemporaneous records and written reports as required.
- Report and proactively support training and development issues arising from clinical and non-clinical incidents.
- Proactively manages risk to vulnerable persons and supports training and development of staff as identified through development plans.
- Participate / facilitate meetings in response to the clinical development needs of staff.
- Work collaboratively with clinical professional colleagues and agencies to ensure effective communication in order that professional practice development needs for staff are identified.
- Manage risk and maintain service delivery through effective communication and liaison with Service Managers, staff and Professional colleagues.
- Engage constructively with clinicians and students to support, develop and enhance quality standards of clinical practice.
- Provide clinical leadership, development and advice to Clinical staff, patients and carers based on current evidence-based knowledge on end-of-life care.
- Partake in clinical supervision of registered professionals as required by the clinical supervision policy.
- Maintain up-to-date clinical skills and knowledge in end-of-life care and awareness of professional issues through appraisal of research and sources.
- Demonstrate personal accountability in everyday practice and responsibility for staff to whom actions are delegated.
- Contribute to improvement of patient care and safety by disseminating and integrating evidence-based guidance into clinical practice across physical health, mental health and learning disabilities.
- Incorporate patient preferences into treatment decisions, such as planning advanced directives near the end of life, including recording on clinical systems RiO, LCR.

Education
- Acts as a clinical and educational expert in end-of-life care, leading in clinical training, education and programmes for the Long-Term Conditions sub-directorate.
- Identifies learning needs for Clinical Professional staff and develops education programmes around end-of-life care.
- Identifies practice areas which require evaluation through team development planning and training needs analysis.
- Effectively manages competing demands of practice and education related to supporting different levels of learning and development.
- Evaluates the effectiveness of education and the impact on service delivery and patient care through audit and benchmarking processes.
- Facilitates learning by actively integrating theory to practice development.
- Proactively creates a positive learning environment within the clinical services which supports lifelong learning, work-based learning and continuing professional development.
- Provides support for learners, mentors and specialist practice students (community nursing pathway) in the education and development process.
- Promotes and facilitates understanding of relevant national and local policies in end-of-life care.
- Responsible for the development of an end-of-life skills data base and the development of career and professional development pathways for this service.

- Maintain clinical skills to practice at a safe level and ensure that the use and application of equipment is in accordance with Trust standards.
- Exercise a high degree of professional autonomy and critical judgement. Make professionally autonomous decisions utilizing knowledge and skills.
- Deliver a high standard of clinical and therapeutic care to patients in accordance with the senior practitioner competencies.
Service Improvement
- The post holder will work closely with the Head of Nursing to support professional recommendations in relation to service redesign and new ways of working.
- Act as a change agent, using skills that will motivate and reassure staff through negotiation and training to facilitate the change process.
- Ensure effective and efficient use of resources, equipment and supplies.

On Call/Unsocial Hours
The service is currently provided from Monday-Friday, excluding bank holidays, 9.00-17.00.

RN Adult
- Registered Adult Nurse, working in a community nursing role at Band 6 for a minimum of 2 years
- Evidence of teaching/training/assessing nursing staff in clinical area
- Enhanced Clinical Assessment skills
- Palliative and EOL Care training or courses.
