A prominent healthcare provider is seeking a Primary and Secondary Care Interface Facilitator in Colchester. East Suffolk and North Essex NHS Foundation Trust is seeking to recruit a well organised, dedicated individual to join us as a Primary and Secondary Care Interface Facilitator based in Colchester.
The closing date is 26 September 2025
This role will be primarily based at Colchester Hospital site, with travel to other ESNEFT sites and GP practices as required.
Within this role you will lead and manage liaison activities between primary and secondary care providers, ensuring seamless, integrated service delivery. You will work to improve communication, streamline referral pathways, promote and facilitate effective use of both primary and secondary care healthcare resources, and promote patient-centred care across the healthcare system.
Excellent communication skills are essential as you will play a key role in facilitating and maintaining effective relationships between primary and secondary care providers.
