Overview

East Suffolk and North Essex NHS Foundation Trust is seeking to recruit a well organised, dedicated individual to join us as a Primary and Secondary Care Interface Facilitator based in Colchester.

The closing date is 26 September 2025

This role will be primarily based at Colchester Hospital site, with travel to other ESNEFT sites and GP practices as required.

Within this role you will lead and manage liaison activities between primary and secondary care providers, ensuring seamless, integrated service delivery. You will work to improve communication, streamline referral pathways, promote and facilitate effective use of both primary and secondary care healthcare resources, and promote patient-centred care across the healthcare system.

Excellent communication skills are essential as you will play a key role in facilitating and maintaining effective relationships between primary and secondary care providers.

Main duties of the job
  • Act as the key liaison between primary care providers (GP practices, community teams) and secondary care (hospitals, specialist services).
  • Build and maintain effective partnerships with clinicians, service managers, and external stakeholders.
  • Lead service improvement initiatives, audits, and evaluations to ensure best practices are shared across care sectors.
  • Analyse referral data, patient outcomes, and other metrics to evaluate service efficiency.
  • Attend local interface meetings, integrated care board (ICB) sessions, and multidisciplinary team (MDT) discussions.
  • Develop and implement strategies to support integrated care pathways.
